Session Overview

Throughout the day, speakers shared examples of how healthcare systems, nonprofits, funders, and public agencies are working together to improve community conditions that influence health. Discussions highlighted practical strategies for sustaining partnerships, using data to guide decisions, engaging community members, and demonstrating impact over time. Participants left with new ideas, connections, and examples they could adapt in their own communities.

    Objectives

    Participants:

    • Identified where sector‑specific goals align with wellness and prevention outcomes.
    • Assessed which forms of cross-sector collaboration are realistic, mutually beneficial, and sustainable.
    • Identified concrete and actionable next steps for program, policy, or partnerships related to chronic disease prevention. 

    Key Themes

    • Meaningful community change is rarely the work of a single organization.
    • Collaborations take time and trust but are key in expanding the work we do.  
    • Clear roles, shared decision-making, community engagement, measurement, and long-term commitment all play a role in sustaining collaborative efforts and creating lasting impact.
    • When it comes to improving community health, results matter, and funders expect that we demonstrate an impact over time. Building capacity and strengthening relationships (“civic muscle”) are also important outcomes that strengthen communities long-term.
