Building Healthier Communities: Planning for Health Equity

Description: In this webinar, the National Association of County and City Health Officials (NACCHO) introduce and review two key tools for internal strategic planning and community health intervention. NACCHO’s Strategic Planning Guide is a step-by-step guide outlining how to prepare for the strategic planning process and implement a strategic plan. The Community Health Improvement Matrix is a tool used to evaluate and create community level interventions to improve population health along the three levels of public health prevention and the social ecological model. NACCHO shares information about both of these tools, defines their importance to addressing social determinants of health and health equity, and suggests applications to participants’ work.
